Get a professional evaluation done so you know that you are getting the right  Mobility Product. Aeroflow employs Mobility Specialists and works with independent physical and occupational therapists to evaluate your needs. We provide your physician with this evaluation so that he or she can make an informed decision on whether or not to prescribe one of these devices.

Post-Discharge OximetryPost-Discharge Oximetry

Aeroflow provides FREE follow-up oximetry screening for patients that have marginal O2 SATS and are weaned off oxygen prior to discharge from the Hospital. We help ensure that your maintaining normal Oxygen Saturation values after you leave the hospital.

  • Free in our service Area. When referred by a physician.
  • An Aeroflow clinician will visit you 3 to 5 days after discharge and perform oximetry at rest, with exercise, and nocturnally.
  • Aeroflow will forward the oximetry report to the appropriate primary care physician so they can determine if further care is needed.
  • Have your Physician write for Aeroflow’s Post-Discharge Oximetry  or let the Case Manager or Discharge Planner know that you want you to have this follow-up screening.

Overnight Oximetry

Overnight Oximetry from Aeroflow is a useful tool to screen for sleep-disordered breathing.
  • COPD is the fourth largest killer of people worldwide.
  • Significant nocturnal oxygen desaturation occurs in up to 45% of patients with moderate to severe COPD who are not hypoxemic while awake.
  • An estimated 18 million Americans suffer from sleep apnea; as many as 90 percent of these people are currently undiagnosed and untreated.

Overnight Oximetry should be considered for patients with the following symptoms/ conditions:

  • Significant smoking history
  • High blood pressure
  • Depression and or change in personality
  • Overweight
  • Morning headaches
  • CHF (Hypoxia contributes to the onset of cor pulmonale)
  • Gasps, snorts, or stops breathing at night
  • Nighttime acid reflux
  • Falls asleep easily during the day even after six or more hours of sleep
  • Excessive daytime sleepiness and/or daytime fatigue
  • Waking with gasping or choking sensations
  • Neck size is 17" or more for a man or 16" or more for a women
  • Snores loudly

Finding out you have Congestive Heart Failure (CHF) can be scary. But CHF just means that your heart does not pump enough blood to meet your body's energy needs. Slow blood flow causes fluid to back up, or become congested, robbing you of energy. You may feel tired, your feet and ankles swell with extra fluid, and simple activities make you short of breath.

How we can help you take charge?

The most important thing you can do for your health is to learn more about your heart failure. Through one-on-one sessions, an Aeroflow clinician will help you understand heart failure, how it affects your life and how you can better manage your disease. We will help you:

  • Understand heart failure and how it affects your body.
  • Learn the signs and symptoms of heart failure.
  • Recognize when your heart failure is worsening by monitoring your weight and how you feel.
  • Discover the importance of limiting salt and fluids.
  • Understand how your medications work.
  • Develop an activity program within your physical limitations.

We will provide you with an Aero-Heart Cook Book, which contains low sodium recipes designed to taste good while also being a part of a healthy diet. We will also provide you with a scale and weight log so you can record your daily weight. In addition to the log, we will give you a medicine schedule to help you take your medicines exactly as prescribed.

How can I enroll in the AeroHEARTS program?

To receive care from the program a referral may be received from your physician's office, hospital staff, or other community services. Once the referral has been received you will be contacted. In addition to program visits, you will need to make sure that you continue to see your primary care physician for all your medical and follow-up care.
